Need a DOT physical performed for your CDL license?

Have the following ready and arrive 10 minutes early for your appointment:
- Driver's License
- CPAP machine results (last 30 days)
- Vision waiver signed by treating physician within 45 days of their DOT physical. This is necessary for conditions such as loss of eye, progressive eye disorders such as macular degeneration, and others.
- SPE (Skill Performance Evaluation) certificate signed by qualified physician prior to examination, certificates generally expire after 2 years. Needed for non-progressive musculoskeletal conditions such as loss of a limb due to an accident.
- For insulin-treated diabetes, bring a ITDM Assessment form signed by treating physician dated within 45 days prior to your examination
- List of medications
- Be prepared to give a urine sample for urinalysis
